Contact Lisa Ferrer, SLOS Director or Connie Wismer, SLOS Assistant Director for available dates. We’ll need information about your students: grade(s), number of students. Also, number of adults coming, preferred season (winter, spring or fall) and idea of classes and/or purpose for your program.

Group Update – Gives us an update on your number of participants (send ASAP)
Parental Release Form – Each student needs a signed form (bring with you)
Adult Release Form – Each adult needs a signed form (bring with you)
School Information Form – Gives our teachers information on your theme and prayer needs (send at least 2 weeks before your program)
Medical/Physical Limitations Form – Gives our staff an idea of what activities are appropriate and/or indication of participation level (bring with you)
Dietary Allergies Form – Helps our Food Service staff know your needs (send at least 2 weeks before your program)
